Job Description
Are you a dependable and flexible caregiver who enjoys both providing hands-on care and being part of an office team? Freedom Home Care is seeking a dedicated Care Team Support Specialist to join our growing organization. This unique hybrid position offers guaranteed 40 hours per week in a dynamic role that bridges the gap between field caregiving and office support.

As a Care Team Support Specialist, you will step in for last-minute caregiver call-outs, help start new client cases, and assist in the office when not in the field. Key Responsibilities
Field-Based Duties
  • Provide in-home care coverage for client visits when other caregivers are unavailable.
  • Cover open shifts due to call-outs, including early mornings, evenings, or weekends as needed.
  • Launch new client cases, ensuring quality care and consistency from the start.
  • Deliver personal care, companionship, and light homemaking according to client care plans.
  • Communicate effectively with the care coordination team about schedule changes and client updates.
Office-Based Support
  • Assist the administrative team with general office duties when not in the field.
  • Support scheduling, documentation, and communication with caregivers and clients.
  • Participate in team meetings and help improve coordination between field and office staff.
Qualifications
  • Prior caregiving experience required; CNA, HHA, or similar certification preferred.
  • Valid driver’s license and reliable transportation required.
  • Ability to adapt to a variable schedule with changing shift times.
  • Comfortable with basic computer use and familiar with scheduling software.
  • Excellent communication, problem-solving, and interpersonal skills.
