Freedom Home Care, a trusted provider of home care services in Southern Minnesota and the Twin Cities area, is seeking a dedicated Client Care Coordinator to ensure seamless scheduling and coordination of care services for our clients and caregivers.
As a key member of our administrative team, the Client Care Coordinator will be responsible for scheduling, communication, and maintaining positive relationships with clients, caregivers, and referral sources. This position requires strong organizational skills, adaptability, and a commitment to excellence in service.
Key Responsibilities
Scheduling & Coordination
- Manage and maintain caregiver schedules to ensure optimal coverage for all client needs.
- Coordinate caregiver assignments for new clients and open shifts, ensuring consistency and compatibility.
- Proactively communicate with caregivers and clients regarding any scheduling changes.
- Monitor caregiver logs daily, making necessary updates for payroll and billing accuracy.
Client & Caregiver Relations
- Foster positive relationships with clients, families, and caregivers to enhance satisfaction and engagement.
- Act as a primary point of contact for scheduling-related inquiries and concerns.
- Provide professional support and encouragement to caregivers, maintaining a positive team culture.
Administrative & Compliance
- Ensure accurate and timely data entry for clients and caregivers in the scheduling software.
- Participate in the on-call/after-hours rotation as needed.
- Answer phones, assist with mail, and perform general office duties as assigned.
- Maintain compliance with company policies, industry regulations, and quality standards.
Qualifications
- Education: High school diploma required.
- Experience: Prior scheduling experience preferred, particularly in home care or healthcare settings.
- Technical Skills: Proficiency in Microsoft Office Suite and scheduling software.
- Communication: Excellent verbal and written communication skills, with a friendly and professional demeanor.
- Organizational Skills: Ability to plan, prioritize, and manage multiple tasks efficiently.
- Flexibility: Comfortable working in a fast-paced environment with frequent scheduling changes.
